CSA Entrepreneur Recognition Award Application

Your application documents must be combined in one (1) pdf file 100MB or under and should include in the following order:

1. Nominator's contact information and nominee's contact information (if different from the nominator).
2. Business Description (1000 words)
   a. A brief description of the business that includes length of time in operation and both the physical address and website or other public digital space address
   b. A brief summary of the business’ target audience or customer base.
   c. Include other material as desired, such as pictures of product, examples of catalogues, brochures or displays that help to showcase the business operation. Maximum 5 images or documents.
3. Entrepreneurial Statement
4. Current Resume for the nominee
5. An abstract of no more than 350 words that describes the vision behind the creation of this business and the goals for moving forward.
6. CSA Value Added Statement
   a. In 350 words or less, describe how you think your work benefits CSA’s mission and membership and why you feel you are a good candidate for this award.
   b. In 100 words or less, state a possible topic for your presentation at the National Symposium. This could be about a product development process, research that led to a service or product, etc. (If you are chosen as the winner of this award, you will have the opportunity to expand your presentation proposal.)
