CSA Howard Vincent Kurtz Emerging Theatre Artist Award Application

A complete application should be uploaded as one (1) pdf document under 100MB and include the following information in listed form:

1. An abstract of no more than 500 words total describing the overall vision of the performed work or research, the applicant's personal statement of inspiration, and a description of challenges discovered in the process of creating the work.
2. Eight (8) clearly labeled images that give a visual overview of the work.
3. An additional four (4) images may be submitted for a maximum of twelve (12) images. These images must be submitted together in one pdf document not more than 25mb.
4. Current resume in PDF format only.
5. Two letters of recommendation from faculty advisors or mentors in the student’s academic program and/or professional arena. At least one of the letters of recommendation must come from a current CSA member. The faculty members may include a design instructor, an instructor or supervisor in shop of studio, history professor or department chair.
